A woman has been sentenced to an indefinite hospital order for the manslaughter of her three children in Bristol, with a judge highlighting failures in her mental health care.

A mother who killed her three young children has been sentenced to an indefinite hospital order following a hearing at Bristol Crown Court. Yasmeen Ali, 44, had pleaded guilty to three charges of manslaughter by reason of diminished responsibility.

The court heard that in February 2024, Ali stabbed her children—seven-year-old Fares, three-year-old Joury, and nine-month-old Mohammed—at their home in Sea Mills, Bristol. Mr Justice Linden, presiding over the case, concluded that the offences were almost entirely attributable to a mental disorder, describing the events as a tragedy that had caused unimaginable pain.

During the sentencing, the court received psychiatric evidence and an independent report concerning the care Ali received from the Avon and Wiltshire Mental Health Partnership NHS Trust. The trust provided a letter to the court apologising for specific failings in the care she was provided, and the judge noted that these deficiencies were linked to the lack of an acute psychiatric bed at a critical time when an assessment under the Mental Health Act had been requested.

The court heard detailed evidence regarding the circumstances leading up to the incident. Ali had been living in difficult financial conditions and expressed fear regarding her safety. Although assessments were conducted in early February 2024, they did not lead to inpatient treatment. The assessment was later cancelled after medical staff observed an apparent improvement in her condition.

Following the tragedy, the Keeping Safe Bristol Partnership has confirmed that a review into the circumstances of the case is currently underway. This report is expected to be published in the coming weeks and aims to identify lessons for agencies and strengthen safeguarding practices across the region.

In his victim personal statement, the children’s father expressed the profound impact of their loss. Mr Justice Linden extended the court's condolences to the family, acknowledging that the defendant herself had been suffering from an undiagnosed schizoaffective disorder at the time of the killings. Ali will now remain in a psychiatric hospital under the terms of the indefinite order.
